Anchorage-Age Membership Application Form
(Please note that if your computer has Acrobat Reader Javascript disabled as currently recommended by Adobe for computer security, these forms will not perform as designed. The latest version of Adobe Acrobat Reader 9.* allows you to enable Javascript for one document which will permit an individual form to work as designed.
The Annual Membership Application form can be viewed and printed with Adobe Acrobat Reader or filled out on line and then printed and signed. The completed form may be mailed to the Anchorage Senior Center or dropped off during normal business hours.
If you don't want to be bothered with annual dues and you are at least 55 years old, you may join as a lifetime member using the Lifetime Membership Form .

What are the membership dues? A single person membership is $40. A married couple or two persons at the same address $70.
